Make a note you can read out loud
- Your own description of the concern you want help with.
- Medicines and supplements you currently use.
- Past diagnoses, allergies, and care you have already tried.
- Any pregnancy, breastfeeding, or pregnancy plans you want to discuss.
- The questions you most want answered.
Bring any records you already have. Ask the service how it wants to receive them; a long upload is not needed just to write your questions.
Ask what comes after the assessment
Medical board guidance calls for the clinician to organise tests or make an examination referral when those checks are needed for the assessment.
This is guidance on providing proper care; the state’s law sets the rules for the visit.
- What question are we trying to answer about my health?
- What information is still missing?
- What options would you discuss, including a plan without medicine?
- If there is a prescription, what is the exact product?
- When and how do we review the plan?
At the end, repeat the plan in your own words. Ask who is responsible for the next step and how to reach that person.
